ITEMS EXCEEDING UPS WEIGHT OR SIZE LIMITATIONS WILL SHIP VIA COMMON CARRIER

Delivery

Orders will be processed the same business day received, providing there are not any type of existing account holds, credit card holds or temporary inventory issues, etc. Orders received by 1PM CST will ship the same business day. Orders received after 1PM CST have a possibility of shipping the same business day or will be scheduled to the next business day depending on order activity (business days do not include weekends or holidays). Our standard shipping method is UPS Ground. Any other shipping method preferred by the customer must be indicated on the order when placed. The customer accepts and is responsible for all freight, insurance, shipping, handling charges and for payment of any and all applicable taxes, with respect to their order. Controll Forms observes the following Holidays and will not process or ship orders on these days; Memorial Day, Fourth of July, Labor Day, Thanksgiving, Christmas Eve, Christmas Day and New Year's Day. Please call Customer Service at 888-219-3501 if you have any questions or any special requests regarding the shipment of your order.

Controll Forms requires notification of any error in shipment by Controll Forms within 15 days of receipt of order by the customer. If you order should contain defective items or incorrect product due to error in ordering by the customer, Controll Forms requires notification within 30 days of order receipt at customer's location. Controll Forms takes pride in utilizing only the best packaging in order to protect our customer's orders during shipping; however, we rely on our customers to inspect incoming shipments in the unlikely event their order should arrive damaged. If the product is damaged in shipment, you have the right to refuse shipment. If you should discover an order error, defective product or concealed damage, please review our Return Policy and contact Customer Service at 888-219-3501. Please have the part number, shipping order and/or invoice number available when you call.

Return Policy

Controll Forms REQUIRES a RMA before any orders can be returned for credit and offers a THIRTY (30) DAY RETURN POLICY. A RMA will be issued only after talking to an authorized Controll Forms Customer Service representative and it is determined the item(s) should be returned. Please contact Customer Service at 888-219-3501 and have the part number or invoice number available when calling.

Once a RMA has been issued, the customer will have 15 days to return the item(s) listed on the RMA. Only the item(s) listed on the RMA will be credited. The RMA number must be clearly marked on the outside of the carton(s). Controll Forms reserves the right at any time to deny any return. Credit will be issued after the receipt and inspection of all item(s) listed on the RMA. All returns must be unopened (other than defective product), in the original packaging and in resellable condition.

Unauthorized item(s) returned to Controll Forms will be returned upon customer request at the customer's expense. No credit will be issued to the customer for the return of unauthorized items. Based upon certain conditions or circumstances, Controll Forms may determine it necessary to apply a restocking fee ranging from 20% to 50% towards the credit for returned orders. The product(s) will be credited to the customer's account through the same method of payment used for the original order.
